Step 2: New Patient Forms
To streamline your visit, please arrive 15 minutes early to your first appointment to fill out the paperwork in person.
Step 3: Consult
During your consultation, you’ll meet with a case manager who will assess your condition and understand your goals. They’ll determine if our services can assist you. If it’s a good fit, you can proceed to an examination to continue your journey towards relief and well-being.
Step 4: Exams
During your first visit, you will meet with our skilled nurse practitioner, who will conduct a comprehensive exam to assess your pain points as well as our experienced DC for a chiropractic exam. They will both gather essential information about your condition. This initial examination helps us better understand your unique needs and provides valuable insights into developing a personalized treatment plan.
Step 5: Creating A Personalized Plan For Treatment
After your initial exam, our experienced team will work together to create a customized treatment plan tailored to address your immediate and long-term needs. We’ll take into account your unique health history, pain levels, and individual goals to design an effective plan that aims to provide you with lasting relief.
Step 6: Report of Findings
At your second appointment, we will review the results from your initial exam in detail. You will have the opportunity to discuss your condition and treatment options with our healthcare professionals.
